However, in the vast majority of cases, the cause is unknown and not attributable to drugs or hormones. I have seen a range of this problem, from small almond-sized collections of breast tissue under the areola to large female-appearing breasts. If the tissue is mostly soft fat, liposuction can effectively flatten the contour of the chest. However, if firm, rubbery tissue is present (usually right behind the areola), liposuction is less effective and I occasionally need to make a small, camouflaged incision at the lower edge of the areola to directly remove the excess tissue. Very rarely, and only in cases of very large breasts and areolas, are more significant incisions needed.
