This patient is a 38-year-old female who presented to my office with complaints of breast size smaller than desired as well as ptosis (drooping) and asymmetry with the right breast being larger than the left. She also was interested in liposuction to improve her contours. I recommended an "addition / subtraction" procedure where I would 1) reduce the larger breast to match the smaller one, 2) lift both breasts and 3) fat graft the breasts to increase upper pole volume and optimize the breast shape.
In surgery I performed a bilateral inverted T scar mastopexy (breast lift) with a small right reduction in addition to power-assisted liposuction of the bra fat, flanks and medial thighs with fat grafting to upper medial poles bilaterally.
Due to a small persistent asymmetry, approximately 40 cc of fat was injected in the right breast and 80 cc in the left, all of which was placed in the superomedial portion of the breast to create better fullness and cleavage in the upper pole. The patient recovered well and was very happy with her improved, low-maintenance breasts.
