This is a 29 year old patient who presented to my Scottsdale practice for a breast augmentation revision. She had previously undergone submuscular breast augmentation elsewhere and was dissatisfied with both the appearance and the feel of her breasts. She had developed severe hyperanimation on both sides, which caused her discomfort and emotional distress. She also had sustained bilateral waterfall deformities (the breast tissue drops and falls off the breast) which made her breasts look oblong and unnatural. To make matters even more complex, she presented with a capsular contracture of the left breast. We discussed converting her new implants to the subfascial plane in oprder to correct her animation deformities combined with mastopexies and placement of Galaform 3D to support the implants in the lower pole.
During her surgery, I removed the capsules, created new pockets in the subfascial plane, and after trying several sizers, placed Sientra 535 cc High Profile smooth round silicone gel implants, both supported by a piece of Galaform 3D bioresorbable mesh for lower pole support. Bilateral mastopexies were performed to both remove the redundant “droopy” breast tissue and to reposition the breast in the appropriate anatomical position above the implant. She is shown postoperatively, feeling extremely happy with her overall cosmetic outcome. Her hyperanimation is completely corrected, and her breasts are the shape and size she has always wanted.
