This is a lovely 32 year old patient who presented to my Scottsdale practice for a possible breast augmentation revision. She had undergone 2 previous surgeries elsewhere, and presented with a recurrent left-sided capsular contracture which caused her pain and discomfort. She also did not like the animation deformity which was associated with her previous submuscular implants. We discussed a site change revision surgery, where her implants would be switched from below the pec muscle to above the muscle (below the fascia) in order to correct her hyperanimation as well as to reduce the likelihood of her capsular contracture. She wanted to be slightly larger but still wanted to appear “natural.”
During her surgery I removed her old implants, capsules, and put her pec muscle back down to the chest wall. New Sientra Moderate Profile 355 cc smooth round silicone implants were placed above the muscle and were supported with Galaform 3D bioresorbable mesh for lower pole support and to diminish postoperative wrinkling. She did not want a circumareolar mastopexy due to the associated scars. She is shown postoperatively, and is thrilled with her result. Her hyperanimation is gone, her breasts are soft and natural looking, and she has achieved the size she always wanted.
