This patient is a female in her early 20's from Texas who flew into my Scottsdale, Arizona area medical practice with interest in breast augmentation surgery. This patient felt she was disproportionately small breasted, and was also unhappy with her "pointy" breast shape.
On evaluation of this patient, I felt that she had a very mild version of tuberous breasts which, in her case, was demonstrated by very tight skin of the lower breasts, wide cleavage, and low set areolas. In order to improve this patient's appearance, I recommended silicone implants to enhance the size and shape of the breasts, internal release of the lower breast tissues to allow the breasts to expand, and circumareolar mastopexy (tightening around the areolas) to "round out" the breasts and raise the areola positions (without detaching the nipples/areolas).
In surgery, I tried multiple sizers and felt that Mentor high profile cohesive silicone gel implants gave the patient the best overall shape. Due to a preexisting asymmetry, I used a 450cc implant on the left and a 425cc implant on the right. By performing the internal breast release and the circumareolar mastopexy, I was able to significantly improve the patient's cleavage and breast shape.
Postoperatively, the patients breasts were very tight due to her natural skin tightness, but over time, the tissues relaxed and within a few months, the patient had a huge improvement in her breast shape and proportion. She is extremely happy with her new appearance, and she and her mother both notice a major boost in her self confidence.
