This patient is a 43-year-old female who had previously undergone bilateral breast augmentation. The patient wished to be smaller and also had malposition of her implants. I felt that implant removal with mastopexy and fat grafting would give the patient the best overall results. The patient wished to have liposuction of the abdomen, flanks and inner and outer thighs, so these were the areas selected for fat harvest.
During surgery the patient's old implants were removed and tissue from the lower breast was repositioned centrally to replace some of the implant volume. Liposuction of the abdomen, flanks and thighs was performed and a total of 170 cc of fat was injected in the right side and 120 cc in the left side due to a mild asymmetry.
Postoperatively the patient recovered well and as seen, the breasts had a much more natural appearance. The majority of the implant volume was replaced by the patient's own natural breast tissue and fat.
