This is a 41 year old Phoenix area male who was unloading an ATV off his truck when it flipped over and struck him in the forehead. He suffered a severe laceration of the forehead with fractures (broken bones) of the outer skull above his right eye (an area called the frontal sinus). As you can see in the preoperative photo, the bony covering of the sinus has flipped over, and is tenting up the skin of the forehead. The sinus cavity itself is visible through the laceration.
I was called in to see this patient at a Scottsdale emergency room, and I brought the patient to the operating room shortly afterwards.
In order to minimize scarring, I repaired all of the fractures through the laceration (skin tear) without needing to make new cuts. I put the broken bones of the sinus back in place and used micro-plates and micro-screws to hold the pieces in place. Once this was done, and the proper contour of the skull was reestablished, I carefully closed the laceration layer by layer using absorbable stitches (the same ones I use for cosmetic surgery) in order to minimize scarring and avoid stitch marks.
The patient healed up without complications, and it is now difficult to see where his injury occurred without looking very closely. He is back to work and has recovered fully.
