The patient is a 50-year-old female who presented to my office and had previously undergone bilateral saline augmentation, which had left her with laterally displaced and severely rippling breasts. I recommended breast revision with exchange to new silicone gel implants, placement of Strattice as an "internal bra", circumareolar mastopexies to tighten the skin, and fat grafting to further hide the rippling issues.
In surgery I performed a bilateral complex revision with exchange from saline to larger silicone cohesive gel implants (560-cc full profile cohesive silicone gel), inferiolateral pocket tightening with Strattice reinforcement, superior and medial capsulotomies, circumareolar mastopexies, power-assisted liposuction of flanks and medial thighs with Puregraft fat grafting to lateral breasts bilaterally.
The patient did very well after surgery and was very happy with her fuller, softer, smoother, more natural appearing breasts.
