The patient is a 61-year-old female who is status post multiple breast operations. The patient had numerous breast augmentation in both the subglandular and subpectoral space. Most recently, she underwent bilateral capsulectomies with implant removal of severe bilateral grade 4 capsular contractures and fat grafting. Due to the patients tremedous tissue damage from her prior surgeries and severe, calcified capsular contracture,the patient developed a significant contour irregularity over the right anterior breast with a severe buckling of the soft tissue in the vertical dimension. At the time of the initial evaluation, multiple operative approaches were discussed including autologous flap salvage surgery vs. tissue expander placement vs. implant placement with concurrent bioresorbable mesh.
In surgery, I used the combination of my aesthetic breast revision surgery skills and my breast reconstruction skills to perform a prepectoral breast reconstruction with a Galaform mesh wrap in addition to a circumareolar (around the areola) breast tightening. I filled out the skin envelope and recreated a soft, natural and symmetrical appearaing breat. The patient was beyond happy with her improvement and felt fully comfortable with her breast apparance for the first time in many years.
